Health Insurance Information

We accept most major forms of health insurance. Be sure to bring your insurance card and a photo I.D. to your scheduled appointment. If you would like to verify we accept your insurance, please call us prior to your appointment at 972-432-8282, or send us a message and we will reply within one business day.

You will be responsible for any co-pays, deductibles, or co-insurance due at the time of service.

If you have HMO coverage, you must obtain a referral from your primary care doctor. Your referral must be current and active prior to your appointment. We can assist you with this process if needed.
