Patient Portal

The Patient Portal is a web portal which allows patients to view their medical records, such as lab results, medical history and medications. Patients can also make online payments using the Patient Portal.

Following the instructions below will provide us with the information necessary for our office to provide the best possible care for you.

Please remember to bring your photo ID and insurance card with you to your appointment.

We ask you arrive at our office 15 minutes prior to your appointment.

To better serve you we would also ask that you send us prior to your appointment (if available):

  • Any previous medical records
  • X-Rays, Ultrasounds, CT or MRI Scans relevant to your appointment (please arrange to have images sent from the imaging center to our office via internet or on a disc before your appointment if possible)
  • Sleep study results
  • Blood work or lab results
  • Any information that would be helpful to assist the doctor with your care.

If your insurance requires a referral, please contact your Primary Care Physician (PCP) prior to your appointment so they may fax it to our office at 972-432-0552. Most doctors require a 48-to-72-hour notice to issue a referral.

You can electronically attest to the Financial Policy, Consent for Treatment Policy, Authorization to Release Information Policy, and Assignment of Benefits using the Patient Portal. These policies are available in PDF form here.

Patient Portal Instructions for New Patients:

  1. Your username will be the email address you provided to the office staff upon registering. Your temporary password will be provided to you by the office staff.
  2. Before starting the Portal process, please email copies of your photo identification (e.g., driver license) and insurance card (if you have insurance) to records@lascolinasentallergy.com.
  3. Upon logging into the Portal for the first time, please reset your password.
  4. Select your preferred pharmacy by selecting the Chart tab, then clicking Personal Details, then Edit, then Add/Search Pharmacies, then Search Pharmacy (click here for a screenshot guide) .
  5. Fill out the available Forms (e.g., Patient Intake Form, Review of Symptoms).

If you are unable to use the Patient Portal to enter your information, you may also fill out the paperwork in the office prior to your appointment.
